Study of functional vessel state in children with juvenile rheumatoid arthritis

This study resolved important scientific and technical problem diagnosis early signs of abnormalities of the blood vessels, namely the development of endothelial dysfunction, increased arterial stiffness and signs of vasospasm and inflammation, autoyimunnyh, metabolic, genetic, autonomic factors and blood pressure, which have a pathological effect on vascular wall in children with juvenile rheumatoid arthritis, in which abnormalities of the blood vessels associated with the early development of atherosclerosis and cardiovascular disease.

The scientific principles and methodology for a comprehensive survey of the functional state by identifying vessels: the endothelial function in the samples with reactive hyperemia (first used coefficient vasodilation index of the brachial artery and types of vessel reaction to reactive hyperemia); local stiffness of the brachial artery; state of the central hemodynamics by ultrasound examination of vessels of the head and neck; C-reactive protein high sensitivity and homocysteine in serum; C677T gene polymorphism metylentetrahidrofolatredutkazy (endothelial dysfunction gene); pathological changes in blood pressure and its daily profile measured at the office and daily monitoring, counting ankle-brachial index (signs of hypertension was found in one third of patients studied); the influence of the autonomic nervous system by registering cardiointervalogramy and evaluation of symptoms of autonomic dysfunction with help of Vaynequestionnaire.

Innovation were design and implementation into work of 3 cardiorheumatology departments and in training at the Department of Pediatrics-2 of the National Medical Academy of Postgraduate Education namely an integrated approach to the diagnosis of the functional state of vessels, including endothelial features and pathogenetic factors and mechanisms affecting it.

The study results were present at international conferences such as the XII International Congress of Medical Sciences (Sofia, Bulgaria, 2013) and the European Human Genetics Conference (Paris, France, 2013).

The economic effect of the introduction  are continuation of patients life, saving their performance, prevent premature death and disability from cardiovascular diseases and their complications, reduce the cost of drug and non drug treatment of atherosclerosis and diseases associated with him in patients with rheumatoid arthritis.

The results of research shows in 10 articles, 4 abstracts  (including 2 in foreign proceedings). Novelty and competitive technical solutions protected by 1 author patent.

The total number of publications of author 15, including 2 abroad.
